The The Family Stability Case Manager provides individualized, strengths-based case management to expectant and single mothers with children participating in Firm Foundation of Virginia programs.
Essential Duties and Responsibilities:
- Conduct participant intake, assessment, and ongoing case management for assigned participants.
- Develop individualized Transition Plans addressing housing, employment and income, financial stability, transportation, childcare, and other identified needs.
- Conduct regular participant meetings and required 30-, 60-, and 90-day progress reviews, with ongoing reviews as required until program exit.
- Identify barriers to housing and economic stability and coordinate appropriate community resources, referrals, and follow-up services.
- Coordinate approved stabilization assistance, including transportation, employment-related needs, childcare-related barriers, household or move-in needs, and other eligible supports.
- Support participants in establishing housing, employment, financial, and family-stability goals and monitor progress toward those goals.
- Connect participants with employment, financial literacy, healthcare, behavioral health, childcare, parenting, education, public benefits, and other appropriate community resources rather than providing all specialized services directly.
- Maintain timely and accurate BIRP case notes, participant records, service documentation, and HMIS when applicable, and other designated systems.
- Provide follow-up and aftercare services as needed to support continued family, economic, and housing stability.
- Communicate significant participant concerns, safety issues, service needs, and barriers to the Executive Director/Program Director and participate in case consultation, supervision, staff meetings, and required training.
Qualifications:
Bachelor's degree in Social Work, Human Services, Psychology, or a related field (required). Master's Degree preferred Minimum of 3 years of case management, counseling, or direct service experience preferred Passion for working with vulnerable populations, especially women and children Strong communication, organizational, and time management skills Familiarity with community resources in the Hampton Roads area Ability to work independently and as part of a team Proficient in Microsoft Office and comfortable with virtual platforms (Microsoft Teams, Zoom, Google Workspace, etc.) Must have reliable transportation, a valid driver's license, and the ability to travel locally
