Job Description

Seeking a fun, energetic, compassionate and caring human services professional to provide case management, outreach, service coordination and support to children and their family members who are experiencing a mental health and/or substance use related issue. Services are provided on an outpatient basis in the office, client homes and in the community. Case Managers are part of a team of professionals including therapists, nurses, vocational counselors and more. This position is full time and is located in our Bradford office. Case Managers have access to a shared company vehicle; use of your own vehicle will also be necessary.
Essential functions include:
  • Providing case management, care coordination, supportive counseling & problem solving to children & their family members.
  • Documenting clinical services in an electronic medical record.
  • Gathering & completing paperwork to open charts, document progress and discharge when appropriate.
  • Providing outreach & support to clients in the community and their homes.
A BA degree in human services, psychology or related field is preferred.
