Case Manager - Residential Substance Use Treatment Veltex Recovery Group | Mount Hope, West Virginia This is a demanding job. Please read the entire posting before applying. Veltex Recovery Group is seeking a Case Manager for our residential substance-use treatment program serving adult women in Mount Hope, West Virginia. We want applicants to understand the position before they apply: this is not a quiet administrative case-management role. Education Requirement A bachelor's degree is required for this position. Applicants who do not possess a completed bachelor's degree will not be considered. Degrees in social work, psychology, counseling, behavioral health, human services, criminal justice, sociology, or a related field are preferred. Relevant professional experience is highly valued, but experience will not substitute for the bachelor's degree requirement. That last sentence is useful because otherwise you will inevitably get applicants asking whether five years of experience can substitute for the degree. Our Case Managers work in a fast-moving residential treatment environment with clients who may have significant legal, housing, transportation, family, benefits, identification, employment, and discharge-planning needs. Many of our clients are involved with courts, probation, parole, corrections, or other community agencies. Priorities can change quickly, deadlines matter, documentation matters, and sometimes several clients need your attention at once. You need to be comfortable with that environment. We are looking for someone who is resilient, organized, resourceful, dependable, compassionate, and capable of solving problems without becoming overwhelmed when the day does not go according to plan. What You'll Actually Be Doing The Case Manager serves as a central point of coordination for the practical and social-service needs that can affect a client's treatment and successful transition back into the community. Responsibilities include coordinating discharge and aftercare planning; communicating with courts, probation/parole, attorneys, community agencies, families and other authorized parties; assisting clients with housing, identification, benefits, transportation and community resources; maintaining accurate and timely case-management documentation; participating in treatment-team coordination; identifying barriers to successful discharge and actively working to resolve them; making referrals and following through until there is an outcome; and communicating significant developments to the clinical team. This position requires follow-through . Sending an email or leaving a voicemail is not necessarily completing the task. We expect our Case Managers to track outstanding issues, follow up, document what occurred, escalate barriers when necessary, and help move matters toward resolution. Can remain calm when a client is frustrated, an outside agency is difficult, or a plan unexpectedly falls apart. Understands that compassion and accountability are not opposites. Does not give up simply because the first solution did not work. There will be complicated discharges. There will be clients who are frustrated with you. There will be outside agencies that do not return your call. There will be days when the plan you spent hours arranging suddenly changes. If that sounds exhausting before you've even started, this probably is not the right position for you. If, however, you are the kind of person who sees a complicated situation and immediately starts figuring out how to solve it, we would like to meet you. We can teach our systems and processes. We cannot teach reliability, resilience, personal accountability, or the desire to follow through for the people we serve.
