Hybrid Essential Duties & Responsibilities Care Management & Member Support Provide person-centered care management and navigation to individuals participating in Social Care Network (SCN) services. Engage individuals to identify health-related social needs, barriers, and appropriate supports. Complete required eligibility assessments, screenings, and other program documentation. Develop and maintain individualized Social Plans of Care based on identified needs. Educate individuals about available services and assist them in understanding and accessing appropriate resources. Provide ongoing follow-up to ensure referrals are completed and identified needs are being addressed. Identify barriers to accessing services and work with individuals and community partners to develop appropriate solutions. These responsibilities reflect the SCN enhanced care management model, which includes eligibility assessment, referral management, development of a Social Plan of Care, coordination of HRSN services, education, and follow-up. Referrals & Resource Navigation Coordinate referrals to appropriate Health-Related Social Needs (HRSN) services and community resources. Assist individuals with navigating federal, state, and local programs and services. Connect individuals to resources addressing housing, food and nutrition, transportation, employment, public benefits, childcare, and other identified social needs. Coordinate with community-based organizations and service providers to support successful referrals and continuity of services. Maintain current knowledge of available community resources and eligibility requirements. Assist individuals with accessing transportation when appropriate to support participation in eligible HRSN services and social care management activities. Transportation is specifically included within the SCN scope for eligible HRSN and social care management activities. Community Outreach & Awareness Conduct outreach throughout the community to increase awareness and utilization of SCN services. Represent United Way at community events, resource fairs, outreach activities, partner meetings, and other community engagement opportunities. Educate community members and partner organizations about available SCN services and how individuals can access support. Conduct presentations and informational sessions for community organizations, healthcare providers, and potential referral partners. Build and maintain relationships with healthcare providers, nonprofit organizations, schools, government agencies, and other community partners. Identify opportunities to reach underserved populations and individuals who may benefit from SCN services. Distribute outreach and educational materials and support community awareness campaigns. Collaborate with United Way staff on outreach strategies designed to increase referrals and engagement. Documentation, Data & Compliance Maintain accurate and timely case notes, assessments, referrals, follow-up activities, and other required documentation. Document participant progress and outcomes in required program and data systems. Track outreach activities, community contacts, referrals, and resulting participant engagement. Maintain confidentiality of participant information and comply with applicable privacy, program, and organizational requirements. Ensure documentation is completed in accordance with SCN requirements and established United Way procedures. Assist with data collection and reporting necessary to measure program performance and outcomes. Community & Partner Coordination Communicate regularly with community-based organizations and other providers involved in participant services. Develop positive working relationships with referral partners and community organizations. Participate in SCN meetings, partner meetings, trainings, and other program-related activities as required. Communicate identified service gaps, recurring barriers, and emerging community needs to the Strategic Impact team. Support coordination between SCN services and other United Way programs and resources when appropriate. General Work collaboratively with United Way staff to support organizational initiatives and community impact goals. Participate in required trainings and maintain knowledge of SCN program requirements and available HRSN services. Maintain professional and respectful relationships with participants, community partners, and colleagues. Perform other duties as assigned. Qualifications Associate's or Bachelor's degree in Human Services, Social Work, Psychology, Public Health, Healthcare, or a related field, or an equivalent combination of education and relevant experience . Previous experience in case management, care coordination, human services, healthcare navigation, community outreach, or a related field preferred. Knowledge of community resources and social service systems throughout the Mohawk Valley preferred. Strong interpersonal, verbal, and written communication skills. Ability to work effectively with individuals from diverse backgrounds and individuals experiencing complex social and economic barriers. Strong organizational and time-management skills with the ability to manage multiple participants, referrals, follow-ups, and deadlines. Strong documentation skills and attention to detail. Proficiency in Microsoft Office and ability to learn required case management, referral, and data systems. Ability to work both independently and collaboratively as part of a team. Reliable transportation and ability to travel locally throughout the Mohawk Valley. Ability to occasionally adjust scheduled hours to participate in community outreach activities, meetings, or events. Work Environment Part-time position averaging 20 hours per week , generally Monday through Friday from 10:00 a.m.-2:00 p.m. This is a hybrid position , with work performed remotely, in the United Way office, and at community locations based on program and participant needs. Local travel throughout the Mohawk Valley region is required for participant meetings, community outreach, resource fairs, partner meetings, and other SCN activities. Occasional evening or weekend hours may be required for community outreach events or program activities, with scheduling adjusted as appropriate. Work involves frequent interaction with program participants, United Way staff, healthcare and social service providers, community partners, and members of the public.
