Social Worker - LMSW PRN Choice Hospice - 4.5 Scottsdale, AZ Job Details PRN 19 hours ago Qualifications Master's degree in social work CPR Certification Master of Social Work Hospice experience BLS Certification Driver's License End-of-life care Full Job Description We are seeking an experienced Hospice Social Worker (MSW) to join our interdisciplinary team and provide compassionate psychosocial support to patients and families navigating end-of-life care. This role delivers coordinated care in the home setting and serves as an advocate, counselor, educator, and resource for patients and caregivers. As a Hospice Social Worker, you will: Complete and document the Comprehensive Assessment within 5 days of hospice admission to evaluate psychosocial, emotional, financial, environmental, and community resource needs. Develop and implement interventions that support patient goals and align with the individualized Plan of Care (POC) . Provide counseling and emotional support related to end-of-life care, grief, loss, family dynamics, and adjustment . Assist patients and families with advance directives, funeral planning, discharge planning, and care transitions . Coordinate community resources and connect families with available support services. Participate as an active member of the Interdisciplinary Group (IDG) through collaboration and care planning. Maintain timely and accurate clinical documentation. Promote dignity, respect, patient rights, and compassionate service throughout the care experience.
Qualifications:
✔ Master's Degree in Social Work (MSW) from an accredited program ✔ Current Social Worker License ✔ Minimum 1 year of Hospice Social Work experience required ✔ Strong understanding of hospice philosophy and end-of-life care ✔ Valid Driver's License and current Auto Insurance ✔ Current AHA CPR Certification HomeCare HomeBase experience preferred
