We are an established CBAS center seeking a Social Worker to deliver compassionate, hands-on support to participants and their caregivers. From welcoming new participants to collaborating with a multi-disciplinary team on individualized care plans, you will play a key part in shaping lives through expertise and understanding. Responsibilities Provides excellent customer service as a full-time staff member on duty during all of the center's hours of service. Orients visitors, families, and new participants to staff, program, and facilities. Obtains data on medical diagnosis and financial situation from participant/care provider as needed to establish eligibility. Completes the Enrollment Process. Develops individual care plans in concert with the interdisciplinary team, incorporating Person-Centered Planning and in conjunction with both participant and family; uses supportive confrontation techniques; identifies and provides treatments/interventions as scheduled on the IPC and documents them the same day they were provided; provides education regarding illness, coping skills, and crisis intervention; refers cases to psychologist/psychiatrist for evaluation (per 42 CFR 441.301(c)(1) through (3)). Completes assessment of home environment prior to enrollment to evaluate living arrangement, accessibility to shopping, doctor, banking, and participant adjustment to surroundings. Communicates regularly with family members; counsels and involves families in treatment at home to enable continuity of care. Provides information and referrals; acts as a liaison between participants/families and social service agencies. Develops individual discharge plans that concur with the interdisciplinary team's assessment; documents changes as prescribed by the MDT team. Maintains accurate and timely documentation of participant and family concerns through progress notes, quarterly reports, and memos as appropriate. Reassesses continuing and returning participants; prepares and submits appropriate treatment plans. Develops and facilitates support groups for participants, family members, and caregivers. Organizes and facilitates outreach programs for participant recruitment; monitors the intake list; finalizes participant enrollment by closely monitoring the process from start to finish. Assists in the development and implementation of in-service and staff training. Provides staff support for external resources and organizes the internal utilization review committee. Performs other duties as assigned by the Program Director. Requirements Applications that do not meet one of the following pathways will not be considered. MSW (Master's in Social Work), or Master's Degree in Psychology, Gerontology, or Counseling from an accredited school, plus one year of experience providing social services in aging, health, or long-term care; OR Bachelor's degree in Social Work from an accredited school, plus two years of experience in social services Other Qualifications Bilingual Mandarin — Required, Cantonese — Preferred Case management experience with the elderly Knowledge of the physical, mental, and social needs of older adults Experience working in a multi-disciplinary team environment Excellent written communication skills and a team-player mindset
