Family Support Services Case Manager Classification:
Full-Time, Non-Exempt Salary Range:
$23.00/hour

Integrity, Respect, Familia, Culture & Community About This Position The Family Support Services Case Manager advances Casa Familiar's mission by providing culturally responsive, client-centered support to students and families, including those referred through school and community partners. Under the direction of the Youth and Senior Services Manager, this position conducts assessments, coordinates care and referrals across multiple systems, advocates for access to resources, and builds trusting relationships that support family stability and student success. Key Responsibilities Comprehensive Case Management Provide deep, individualized case management to clients facing multifaceted challenges including housing instability, mental health needs, and economic hardship. Conduct thorough assessments to identify client strengths, needs, and goals and design action plans that will effectively address client needs. Develop and implement tailored service plans in collaboration with clients, ensuring alignment with their values and priorities. Maintain accurate and timely documentation of case notes, service plans, and progress reports. Client Advocacy & Support Navigate complex systems on behalf of clients, including public benefits, legal services, healthcare, and housing. Serve as a liaison between clients and service providers to ensure coordinated care and continuity of services. Advocate for client rights and access to culturally appropriate resources. Respond to crises with empathy and professionalism, connecting clients to emergency services when needed. School & Family Support Services Serve students and families referred through Sweetwater Union High School District (SUHSD) and other designated community partners. Conduct family assessments and provide referrals, resource navigation, and follow-up support based on identified needs. Connect students and families to rental and housing assistance, food resources, medical services, after-school programs, internships, and other community-based supports. Conduct home visits with parents/guardians of students experiencing high absenteeism, as appropriate. Participate in school-based meetings and coordinate with designated school and district personnel to support students and families. Maintain timely case documentation and assist with required program reporting, including services provided, referrals, outcomes, and other required grant or contract metrics. Program Oversight & Collaboration Support the implementation of community-based programs that serve high-need populations. Collaborate with program staff to ensure services are trauma-informed, equitable, and responsive to community feedback. Participate in program planning and evaluation activities to help improve service delivery and outcomes. Collaborate with the Program Manager and relevant Casa staff to strengthen service delivery and identify emerging needs of students and families. Reporting & Compliance Track client outcomes and program metrics to inform continuous improvement while prioritizing client confidentiality, continuous case notes, and help build ways to improve unique identifiers and categories of service delivery. Ensure compliance with grant requirements, ethical standards, and organizational policies. Work with the program manager on budget tracking and resource allocation to support client needs. Community Engagement Build and maintain relationships with community partners, service providers, and stakeholders. Represent Casa Familiar at outreach events, coalitions, and advocacy initiatives. Promote Casa's programs and services to increase visibility and impact. Education & Experience Bachelor's degree in social work, psychology, public health, or a related field preferred. Experience in case management, family support services, or direct service with high-need populations. Experience with Salesforce or a similar case management system preferred. Qualifications & Skills Bilingual, bicultural, and biliterate in Spanish and English required. Proficiency in Microsoft Office (Word, Excel, PowerPoint), online databases, and project management tools. Strong interpersonal and communication skills, with the ability to build relationships with diverse students, families, community partners, and stakeholders. Knowledge of, or commitment to developing a strong understanding of, trauma-informed care, harm reduction, and systems navigation. Strong organizational and time-management skills, including the ability to manage multiple cases, follow-up activities, and documentation deadlines. Ability to work independently and collaboratively in a fast-paced, resource-limited environment while adapting to changing client needs. Valid California driver's license and reliable transportation required due to travel between school, community, and home-visit locations. Physical Requirements & Work Conditions This position requires sitting at a desk or workstation, walking, standing, climbing stairs, hearing, speaking, and the ability to lift up to 25 lbs. Regular local travel between Casa sites, schools, community locations, and occasional home visits may be required. Occasional evening and weekend work may be required depending on the organizational calendar. A background check is required.
