Duties Conduct thorough psychosocial assessments to evaluate patients' emotional, social, and behavioral needs across diverse healthcare settings. Develop and implement individualized care plans aligned with clinical ethical guidelines and social work practice standards. Provide evidence-based clinical counseling, including behavioral therapy, motivational interviewing, and group therapy to support mental health and behavioral health goals. Coordinate community resources such as hospice care, assisted living, and home & community care services to ensure comprehensive patient support. Assist with discharge planning by preparing patients for transition from inpatient to outpatient or home care environments, ensuring continuity of care. Maintain accurate medical documentation in compliance with HIPAA regulations and hospital policies, including medical records and CPT coding where applicable. Collaborate with interdisciplinary teams—including medical professionals, nurses, therapists, and case managers—to deliver holistic patient-centered care. Experience Proven experience working as a social worker in acute care, inpatient settings, nursing homes, or outpatient clinics with a focus on medical social work. Demonstrated expertise in child & family counseling, hospice & palliative medicine, dementia care (including Alzheimer's), PTSD Care, and working with geriatric patients. Familiarity with clinical research methodologies, data collection processes, ICD coding systems (ICD-9/ICD-10), and CPT coding for documentation accuracy. Strong background in crisis intervention, mental health counseling, diagnostic evaluation, case management, and client communication. Experience working with individuals from diverse cultural backgrounds and those with disabilities or autism spectrum disorders. Knowledge of medical terminology related to hospice care, end-of-life care, assisted living environments, post-acute care settings, and inpatient hospital experience is highly valued. Ability to manage complex cases involving behavioral health issues such as PTSD or dementia while adhering to social work practice standards and legal requirements like HIPAA.
