  • POSITION SUMMARY
  • The Case Manager assists patients and families along the care continuum with social and emotional issues that may affect their medical care and well-being.
Case manager is responsible for case management, healthcare system navigation, as well as education and coordination of available and relevant community resources and referrals. Serves as a CMS-designated qualified health care provider completing services, documentation, and insurance-based requirements. Works with patients, families, physicians, staff, inter-hospital disciplinary team, and external community partners to facilitate plan of care.
JOB SPECIFIC DUTIES
1. Collaborates with provider, patient, family and inter-disciplinary team to develop a comprehensive and integrated patient needs assessment and develops plan of care with patient and/or family to meet those needs as appropriate. 1. Initiates and conducts psychosocial assessments as needed or as directed by providers to identify social, economic, functional and behavioral barriers with an emphasis on patient self-determination/self-care and to promote and enhance optimum health outcomes. 1. Prepares, coordinates and completes follow up on community resources and ordered referrals. i.e. DME equipment, home health, community elder programs. 1. Conducts initial and ongoing assessment of at-risk patients as needed or directed by provider. Communicates and collaborates with provider on care options. 1. Completes abuse/neglect/exploitation screenings and completes state-required reporting as appropriate. 1. Counsels and supports within scope of practice, with regards to social/emotional needs of patients/families/caregivers. 1. Serves as CMS-designated qualified health care provider conducting Medicare/insurance covered services appropriate to the role. Examples include advance care planning, screenings and counseling services defined by CPT code guidelines. 1. Performs and documents appropriate patient screening tools as ordered and communicates results to provider. Examples are MMSE, MOCA, Geriatric Depression Scale, PHQ Depression Screening, clock, etc. 1. Assesses, collaborates, and assists with coordination of treatment plan options correlating to a patient's change of status, i.e. initiation of home care, nursing home, assisted and skilled living, Hospice, etc. 1. Initiates and conducts family and patient conferences as appropriate. 1. Reviews and evaluates the care plan, updates and documents the patient's progress toward resolution of problems at each visit or as appropriate. Solicits feedback from caregivers as appropriate as to patient's response to psychosocial intervention. 1. Engages patient and family in educational opportunities about patient care/caregiver needs and providers information about relevant community activities and resources. 1. Provides education/explanation, and guides discussion with patients, families and care team on continuum of care issues such as, but not limited to advance directives, healthcare wishes, patient rights, long term care placement, palliative care path, end of life planning. 1. Provides educational counseling about health insurance resources, referrals and assistance as needed related to medical bills/unmet care needs. 1. Completes data collection, prepares reports, proposals, and shares patient statistics for service line enhancements. 1. At the discretion of SHC leadership presents in-services and training for staff and/or community audiences. 1. Responds promptly and accurately to meet patient's needs. 1. Maintains quality care monitors for positive patient outcomes
