Responsibilities Conduct thorough patient assessments, including psychosocial evaluations and medical history reviews, to inform individualized care plans. Coordinate discharge planning by collaborating with healthcare teams, families, and community resources to ensure smooth transitions from inpatient to post-acute or outpatient settings. Manage clinical case documentation while maintaining compliance with HIPAA regulations. Provide behavioral health support and mental health counseling for members with diverse conditions, including mental health diagnoses. Facilitate communication between patients, families, healthcare providers, and community agencies to optimize care delivery and resource utilization. Facilitate individual sessions, workshops and groups. Qualifications Knowledge clinical case management principles. Experience working with children, individuals with disabilities, mental health conditions, or substance use disorders is highly desirable. Excellent client communication skills along with the ability to work effectively with diverse cultural backgrounds and family units.
