Simplicity Home Care is growing in the Boise area, and we are looking for a compassionate, driven Social Worker to help us serve local veterans and their families. This is more than managing a caseload. You will have the opportunity to build real relationships with veterans, advocate for their needs, solve problems, connect families with resources, and help veterans remain safely and independently in the homes they love.

What You'll Do As a Social Worker/Veteran Case Manager, you will become one of the primary points of support for the veterans and families we serve. You will work closely with veterans, caregivers, families, the VA, and our internal team to make sure each veteran receives exceptional care.
Your responsibilities will include:
Build meaningful relationships with veterans and their families Advocate for veterans and help identify resources that can improve their quality of life Conduct assessments and home visits Develop and update individualized care plans Maintain regular contact with veterans and their families Identify changes in a veteran's condition, environment, or needs and take action Help coordinate caregivers and services Coach, mentor, and support caregivers Work with families, community resources, and VA partners to solve problems Respond quickly when a veteran or family needs assistance Review care documentation and ensure quality standards are being met Participate in team meetings and ongoing care planning Who We're Looking For We're looking for someone who genuinely enjoys helping people and isn't afraid to take ownership when something needs to get done.
