We are seeking a Psychiatric Rehabilitation Service Coordinator (PRSC) to whom the resident primarily relates for the coordination of services, programs, oversight of data collection and review of resident performance. Our ideal candidate must have a bachelor's degree in psychology, social work, or another human services / social services discipline.

Qualifications Include:

Bachelor's degree in psychology, social work, or related social services / human Services field such as sociology or special education. One year working with the adult mentally ill population; preferred but not required. Prefer experience with caseloads, crisis intervention, documentation, care planning with involving families in the process, knowledge of the MDS, discharge planning, facilitating skills groups and psycho-social groups and problem solving. Good assessment skills, interpersonal skills, advocacy skills, and communication skills. Prefer knowledge of MDS 3.0 along with basic computer skills. Charting experience preferred. Must be able to work in an interdisciplinary team environment. Must be able to handle confidential data with professional discretion. Position is 5 days/weeks 8 hours per day.
