SOCIAL WORKER - Discharge Planning Team

Job Summary:

Position Summary:

In direct delivery of patient care, and under the supervision of the VP of Operations, the Licensed Social Worker is a collaborative member of the Care Management Team. In addition, the Licensed Social Worker serves as the consistent point of contact for referring physicians, patients, caregivers, families, home health, and community. The licensed social worker will establish good working relations with organizations within the community.

Position Responsibilities:

The Licensed Social Worker will collaborate closely with the interdisciplinarity team to function as an advocate for patients, family, and caregivers to communicate patient's wishes to the treatment team. A Licensed Social Worker will work with patients to identify the need for support regarding social determinates of health in order to facilitate appropriate community resources to mitigate risks to patients as they transition back to the community.

Comp Range:
