Job description Downriver Medical Associates looking for a Licensed Medical Social Worker to act as an advocate and help people foster their capacities and coping mechanisms in order to improve and better their daily lives and experiences, within specified standards and timescales.
Summary:
The Health Care Social Worker provides emotional, psychosocial, and resource support to patients navigating medical, mental health, and social challenges. This role assists patients in accessing healthcare services, community resources, and information related to their health conditions while promoting overall well-being and recovery. Responsibilities include conducting patient assessments, providing counseling and supportive interventions, and connecting patients with social services such as housing assistance, financial resources, support groups, transportation, and employment opportunities. The Health Care Social Worker serves as a patient advocate, educator, advisor, and care coordinator to help ensure comprehensive, patient-centered care. This position works collaboratively with psychologists, primary care providers, and other members of the interdisciplinary healthcare team. Responsibilities also include assisting with telepsychiatry visits, documenting patient encounters in the electronic medical record, and carrying out care plans and provider orders as directed by the primary care team and consulting psychologist.

Qualifications:
Must have active MSW in State of Michigan Previous experience of at least 1 year as a social worker.
Physical Requirements:
  • May be expected to lift up to 50 pounds or 150+ pounds with assistance. May be expected to push or pull 150-300 pounds with assistance.
  • Work is of medium demand
  • Optimal visual and auditory acuity are essential to performing designated duties required. Optimal manual dexterity involving the handling of medical equipment and/or instruments.
  • Physical conditions are clean, neat and well-lit.
  • Hours of duty may be irregular or unexpectedly extended due to the requirements of the job.
Duties and Responsibilities:
  • Developing a health care plan for clients, such as a treatment and discharge plan
  • Educating patients and their families
  • Managing client treatment routines and handling crises as they arise
  • Conducting comprehensive screenings for incoming patients to assess their needs
  • Organizing or referring patients to community support groups
  • Offering advice and counseling services to their clients
  • Coordinating home care, medical equipment rentals and meal delivery
  • Interviewing patients and families to determine a patient's needs
  • Helping patients receive admission to inpatient and outpatient services or obtain funding for treatment or therapy
  • Collaborating with other health care experts to assess patient needs
  • Maintaining and updating client records and completing paperwork
  • Providing emotional support to patients' families
  • Advising on health care services, policies and legislative challenges
  • Advocating for policies that reduce health disparities and improve access to health care
  • Demonstrates by example commitments to: o Physician and patient customer service o Continuing quality improvement o Promotion of patient and staff safety o Protection of confidential patient and staff information o Growth of appropriate utilization o Containment of controllable expenses o Compliance with applicable federal, state and local laws
  • Document examination findings
  • Place orders for outside exams and referrals.
  • Help keep patient flow maintained.
  • All relevant and related duties as required.
The salary range for this position is $70,000 to $80,0000 annually , commensurate with the candidate's skills, experience, education, and overall qualifications. We seek candidates who possess the knowledge, technical abilities, and relevant experience necessary to perform the responsibilities of the role effectively. Candidates with additional specialized skills, certifications, or demonstrated expertise may be considered for compensation within the higher end of the salary range.
Pay:
$70,000.00 - $80,000.00 per year
