The role includes assessing emotional, social, environmental, and caregiver needs; developing and implementing individualized interventions; providing counseling and support related to illness, end-of-life, and grief; connecting patients and families with community resources; advocating for patient and family needs; and collaborating closely with the interdisciplinary team to promote comfort, dignity, quality of life, and goal-concordant care. The Field Social Worker independently manages an assigned caseload, maintains timely and accurate documentation, participates in IDG meetings, and communicates changes or concerns to the appropriate members of the care team.
