Duties Conduct thorough assessments of clients' needs, focusing on admission/discharge and behavioral health issues Develop individualized care plans. Provide intervention and management of resident psychosocial needs,admission and discharge needs being met. Collaborate with multidisciplinary teams to coordinate home health for discharge needs. Advocate for resident rights within the nursing home and facilitate communication between residents,their families and service providers Maintain detailed documentation of client interactions and progress notes in PointClickCare Deliver client communication with sensitivity and clarity, ensuring culturally competent support for diverse communities Maintain HIPPA regulations. Requirements Ability to work with geriatric population and their families. Communication skills combined with writing abilities to document cases accurately and professionally.
