Job Overview We are seeking a compassionate and dedicated Social Worker to join our dynamic team, committed to empowering individuals and families through comprehensive social services. In this vital role, you will provide support, advocacy, and intervention for our geriatric population. Your expertise will help foster positive outcomes by addressing complex needs related to admissions, behavioral health, and family dynamics. The ideal candidate will bring energy, empathy, and a proactive approach to creating safe and supportive environments for those we serve. Duties Conduct thorough assessments of clients' needs, focusing on admission/discharge and behavioral health issues Develop individualized care plans. Provide intervention and management of resident psychosocial needs,admission and discharge needs being met. Collaborate with multidisciplinary teams to coordinate home health for discharge needs. Advocate for resident rights within the nursing home and facilitate communication between residents,their families and service providers Maintain detailed documentation of client interactions and progress notes in PointClickCare Deliver client communication with sensitivity and clarity, ensuring culturally competent support for diverse communities Maintain HIPPA regulations. Requirements Ability to work with geriatric population and their families. Communication skills combined with writing abilities to document cases accurately and professionally.
