Job Overview We are seeking a compassionate and organized Hospice Social Worker (BSW) to join our interdisciplinary hospice team in Butte, Montana. This role serves as the primary social worker for assigned hospice patients, providing psychosocial assessment, care coordination, emotional support, and advocacy throughout the end-of-life journey. This is a full-time position working a varying schedule. We offer a competitive wage range of $25.00 to $37.00 per hour depending on years of experience and qualifications. The Hospice Social Worker is a core member of the Interdisciplinary Group (IDG) and delivers services in accordance with CMS Hospice Conditions of Participation (§418.56) and ACHC standards. Key Responsibilities Social Work Case Management Manage an assigned hospice caseload as the designated social worker Complete psychosocial assessments as part of the hospice comprehensive assessment Identify emotional, social, financial, environmental, and caregiver needs Reassess psychosocial needs as patient condition and family dynamics change Patient & Family Support Provide counseling, emotional support, and crisis intervention within BSW scope of practice Support caregivers with coping strategies, anticipatory grief, and end-of-life education Assist patients and families with advance care planning and end-of-life planning Serve as an advocate for patient and family goals of care Care Coordination & IDG Participation Actively participate in interdisciplinary group (IDG) meetings Collaborate with nursing, chaplain, physician, and hospice leadership Communicate psychosocial findings and recommendations to the team Coordinate referrals to community resources and supportive services Documentation & Compliance Complete accurate and timely documentation of all visits, assessments, and interventions Ensure documentation meets CMS Hospice CoPs (§418.56) and ACHC requirements Maintain compliance with hospice policies, HIPAA, and ethical standards Complete psychosocial assessments as part of the hospice comprehensive assessment Identify emotional, social, financial, environmental, and caregiver needs Reassess psychosocial needs as patient condition and family dynamics change Qualifications Required Bachelor's degree in Social Work (BSW) from an accredited program Eligible for or holding state social work licensure/certification, as applicable Ability to function independently as a hospice social work case manager Strong organizational, communication, and documentation skills Reliable transportation and valid driver's license Preferred Hospice, palliative care, or home health experience Experience managing a patient caseload in a healthcare setting Knowledge of grief, loss, and end-of-life psychosocial care What We Offer Competitive pay based on experience Full-time benefits package Supportive interdisciplinary team Meaningful, patient-centered work Ongoing training and professional development Why Join Us Our hospice team is dedicated to providing compassionate, compliant, and holistic end-of-life care.
