Hospice Social Worker Job Overview We are seeking a compassionate and dedicated Hospice Social Worker to join our healthcare team. Provide counseling and emotional support to patients facing life-limiting illnesses, helping them cope with grief, loss, and other psychological challenges. Assist patients and families with navigating healthcare resources, community services, and advance care planning. Coordinate with healthcare providers to ensure holistic patient care and facilitate communication among team members. Advocate for patient rights and preferences within the hospice setting. Document all interactions, assessments, and care plans accurately in accordance with organizational policies. Support families through grief processes and bereavement services as needed. Experience Proven experience in medical social work or a related healthcare setting, preferably within hospice or palliative care. Strong understanding of psychosocial issues faced by terminally ill patients and their families. Excellent communication and interpersonal skills to build trust and rapport with diverse populations. Ability to work collaboratively within an interdisciplinary team. Knowledge of community resources, healthcare laws, and ethical standards related to hospice care.
