Social Services Director - Full-Time 1st Shift BrookHaven Retirement Community Brookville, OH Brookhaven Retirement Community
LSW REQUIRED
Position Summary:
Ensures the emotional well-being of each resident in order to function within the highest quality of life.
Essential Functions:
Because BrookHaven exists to care for our residents, regular attendance and punctuality are essential to provide quality care
Leadership:
Ability to take action and achieve results through others Commitment to BrookHaven mission, values, philosophy and Service Excellence Initiative. Must be able to complete performance requirements independently while using good judgment as well as have the ability to harmoniously work with others Must be able to read, write and understand the English language
Position Responsibilities:
Must follow BrookHaven Policy and Procedures Possesses knowledge of policies and procedure for social service, Level of Care and PASSAR screens Identifies psychosocial needs and interests of the resident upon admission and on-going Completes sections of the MDS and develops care plans that are specific to each resident Acts as a Resident Rights advocate, including providing staff education on resident rights and abuse Implements and participates in discharge planning and documentation; Discharge notes are to be completed in a timely manner Provides services through referrals and communication with outside agencies Demonstrates ability to gather and utilize information within the electronic health record systems Participates in the QAPI process Timely completion of Social Service assessments, Social Histories, completion of Plan of Care, update as needed, attend care conferences as scheduled, and complete progress notes. Assumes responsibility for maintaining his/her own professional competence Share understanding of social and emotional factors with other staff members by means of records and conferences Specifically document via
IDT, POC, CAA
related to psychotropic medications, devices, code status, hearing, dental and vision needs Schedule and manage ancillary services - Dentist, podiatrist, eye Dr, etc visits Perform other duties as assigned
Knowledge, Skills and Abilities:
Point Click Care knowledge preferred. Associates degree or equivalent, Bachelor Degree preferred. Licensed Social Worker (LSW) in Ohio.
