A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. No employer-provided benefits Position Summary Bend Senior Care Management is seeking an experienced, compassionate Social Worker to provide individualized care management and advocacy for older adults and their families. This position requires a highly dependable and flexible professional who can adapt to changing client needs, medical appointments, family requests, and unexpected situations. Because our clients' needs can change quickly, the ability to adjust your schedule when necessary is an essential part of this position. Schedule & Compensation Approximately 32 hours per week $40-$45 per billable hour , based on experience, education, and qualifications Must have a flexible schedule and be willing to adjust hours based on client needs Some appointments, meetings, and client needs may occur outside of a traditional 9-5 schedule Local travel throughout the Bend area is required No employer-provided benefits are offered This position requires reliable availability and the ability to respond appropriately when client needs change unexpectedly Key Responsibilities Conduct comprehensive assessments of older adults and identify care needs, risks, and available resources. Develop and implement individualized care plans. Coordinate medical, home-care, hospice, rehabilitation, assisted living, memory care, and other services. Advocate for clients and communicate with physicians, healthcare providers, families, and facilities. Attend medical appointments, care conferences, and facility meetings as needed. Assist families with difficult decisions related to aging, safety, cognition, living arrangements, and changes in care. Monitor clients' changing physical, cognitive, emotional, and social needs. Respond appropriately to urgent or changing client situations. Maintain accurate and timely documentation. Build strong, trusting relationships with clients and their families. Maintain confidentiality and professional boundaries.
Qualifications Required:
Master's degree in Social Work or a related field. Experience working with older adults. Strong communication, organizational, and problem-solving skills. Ability to work independently and exercise sound professional judgment. Excellent interpersonal skills and a compassionate approach. Demonstrated flexibility and willingness to adapt to changing client needs and schedules. Reliable transportation and valid driver's license. Ability to work in clients' homes, healthcare facilities, and community settings.
Preferred:
MSW and/or LCSW. Experience in geriatric care management, hospital social work, skilled nursing, assisted living, memory care, hospice, or home health. Experience working with dementia and complex family dynamics. Knowledge of Medicare, Medicaid, long-term-care insurance, and community resources.
