Social Worker/Care Companion Carease Health At Carease Health - we prioritize proactive measures for enhanced physical health and well-being through our comprehensive preventative care program, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM) and Collaborative Care Management (CoCM). We are seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team. Key Responsibilities As a Social Worker/Care Companion, you will visit designated facilities, conduct assessments of residents to identify emotional, cognitive, and psychosocial needs, and implement short-term, solution-focused therapy and supportive interventions to address immediate concerns. You will also document visit notes and maintain detailed records of interactions and interventions after each visit, collaborate with psychiatric professionals, and participate in a monthly meeting with the psychiatric NP. Qualifications We are looking for candidates with an Associate's degree in Social Work, Psychology, Counseling, or another mental health-related field, and at least 1 year of experience. You should have a strong understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings, as well as strong communication, documentation, and relationship-building skills.
