Social Worker Position Summary:
The Social Determinants of Health (SDOH) Social Worker provides behavioral health assessment, individualized care planning, and counseling to individuals and families experiencing or at risk of homelessness. Using trauma-informed, person-centered, culturally responsive, and evidence-based practices, this role addresses barriers to housing stability and connects clients with behavioral health, housing, and community resources. The Social Worker serves as the primary liaison with Community Care Behavioral Health, coordinating referrals and timely access to services, and supports service continuity across agency departments, including Coordinated Entry, as needed. Primary Duties and Responsibilities Assessment and care planning: Assess clients' behavioral health and social service needs, prepare behavioral health summaries, and develop individualized Plans of Care that reflect client goals, strengths, supports, and barriers to stability.
Counseling and intervention:
Provide trauma-informed, evidence-based individual, family, and group counseling, including short- and long-term interventions addressing emotional well-being and substance use prevention.
Client engagement and support:
Conduct outreach, encourage program participation, and provide resources, supportive services, and planned or unscheduled assistance to address clients' and families' needs.
Service coordination and partnerships:
Serve as the primary liaison with Community Care Behavioral Health; coordinate referrals and access to housing, behavioral health, medical, educational, vocational, and other community services.
Safety and crisis response:
Conduct safety assessments and provide crisis response and behavioral stabilization services to support client safety and stability.
Documentation and compliance:
Maintain accurate, timely case notes, care plans, correspondence, and reports in accordance with agency, program, funder, and applicable HMIS requirements.
Collaboration and professional development:
Participate in case reviews, staff meetings, supervision, agency events, and required training; complete applicable continuing education requirements and training on agency systems.
Community and agency support:
Travel frequently throughout Delaware County to provide community-based services, support other departments—including Coordinated Entry—as needed, and perform other duties as assigned. Qualifications A Master of Social Work (MSW) and at least three years of experience in social services working with individuals experiencing homelessness, mental health conditions, and/or substance use disorders are required. Candidates must demonstrate strong assessment, care planning, and service coordination skills, along with knowledge of Progressive Engagement principles. Experience applying trauma-informed and strengths-based approaches is preferred. Excellent oral and written communication skills, sound interpersonal judgment, and the ability to work effectively with individuals from diverse cultural and socioeconomic backgrounds are essential. Knowledge of Delaware County's social service network and the ability to maintain productive relationships with community providers are preferred. A company vehicle will be available for work-related travel. Candidates must possess a valid driver's license, maintain automobile insurance, and have access to reliable personal transportation.
Pay:
$56,000.00 - $65,000.00 per year
