Requirements:

Looking for someone with medical social work background that has experience with d/c planning and preferably someone with behavior health inpatient experience BLS (AHA) only Master's Degree Social Work Required PA License Required. Clinical Social Worker Certificate. Overview We are seeking a dedicated Licensed Social Worker to join our multidisciplinary healthcare team. This role involves providing comprehensive psychosocial assessments, counseling, and support to patients and their families across diverse settings, including hospitals, outpatient clinics, hospice, and community-based programs. Responsibilities Conduct thorough patient assessments to identify psychosocial needs, including mental health status, family dynamics, and social determinants of health. Provide individual, group, and family counseling sessions utilizing evidence-based practices such as cognitive behavioral therapy, motivational interviewing, and behavioral therapy. Coordinate with healthcare teams to develop and implement discharge plans that address medical and social needs. Assist patients with navigating community resources related to hospice care, assisted living, disability services, and other support systems. Document all clinical interactions accurately within electronic medical records (EMR) systems while adhering to HIPAA regulations and medical documentation standards.
