Job Summary The Hospice Social Worker is essential for meeting the psychosocial needs of hospice patients and their families, including assisting with end of life planning, support for options at time of death, and assisting with options as patient care needs increase. Responsibilities

• Assess hospice patients and families/caregivers to identify psychosocial, financial, environmental, and community needs as evidenced by plan of care documentation, clinical records, and community resource referrals
• Provide effective interpersonal communication skills to help facilitate complex conversations with patients, families, caregivers, and community members
• Meet mandatory continuing education requirements, per Oregon state requirements
• Attend interdisciplinary team meetings
• Communicate with the case manager and provider team to convey care needs that may need medical intervention
• Accept clinical assignments that are consistent with the education and competence to meet the needs of the patients
• Actively participate in quality assessment performance improvement teams and activities
• Accept and perform other related duties and responsibilities, as required Requirements
• Must be a graduate of an accredited social work program
• Must have an active social work license in Oregon

Pay:

$40.00 - $47.00 per hour
