You will complete resident assessments and review care plans alongside floor nurses. The daily workflow centers on direct communication with facility staff and family contacts to keep stay transitions on track.

Compensation, Schedule & Benefits Pay Rate:

Starting at 35/hr, scaling upward with experience

Schedule:

Monday to Friday, 8AM - 5PM (Full-Time)

Qualifications & Requirements Active Social Worker License (LSW, LBSW, LMSW, or LCSW) in the State of Texas Prior social work experience in a skilled nursing facility or long-term care setting preferred Working knowledge of Medicare, Medicaid, and discharge planning processes Must pass a standard criminal background check Must pass a drug screening Reliable transportation for consistent attendance Ready to Join Our Team?
