A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Rotating | PRN How you'll make an impact in this role: Design personalized care plans across the healthcare continuum by integrating patient and family values, ensuring every patient receives compassionate, holistic care tailored to their life goals. Provide vital crisis and psychosocial support by evaluating and treating complex social determinants of health—such as substance abuse, trauma, and adjustment needs—to stabilize patients during critical care transitions. Champion patient advocacy by proactively identifying and dismantling financial, social, or systemic barriers so patients gain seamless access to essential community resources. Collaborate with care transition teams to maintain real-time alignment on health plan progress, driving continuous, high-quality care adjustments throughout the patient's journey. Guide complex post-discharge strategy and documentation, serving as the subject-matter expert on level-of-care recommendations and post-acute services aligned with clinical diagnoses. What minimum qualifications you'll need: Licensure / Certification /Registration:
One or more of the following required: Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date. Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred .Education:
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required. Life atAscension:
