The Licensed Clinical Social Worker provides concrete and supportive services to hospitalized patients and their families, including psychosocial assessment, individual and/or family counseling and crisis intervention. This position treats/cares for patients of all ages, and duties can include: Discharge Planning Services to achieve hospital goals of Quality Care and cost containment. Acting as a liaison to relevant community agencies. Providing Case Management Services, both within the hospital and in the community, to a capitated patient population with responsibility for facilitating access to care, coordinating care in medically complex situations and appropriate utilization of financial resources. Participating in Hospital Team Conferences. May provide social work student supervision, in-service presentations in areas of social work expertise for hospital and community agencies, and may facilitate groups for staff, patients or families.
