Job Description

Jackson Therapy Partners is seeking a Travel Licensed Clinical Social Worker (LCSW) to provide compassionate, patient-centered behavioral health support across diverse healthcare settings nationwide. In this travel role, you'll complete psychosocial assessments, deliver individual and group counseling, coordinate discharge and community resources, and collaborate with interdisciplinary teams to support optimal patient outcomes. Responsibilities Provide comprehensive psychosocial assessments for patients and families across diverse healthcare settings. Deliver individual, family, and group counseling to address adjustment to illness, behavioral health, trauma, grief, and coping needs. Perform crisis intervention, including risk assessment, safety planning, and coordination of higher levels of care when necessary. Develop and implement discharge and transition plans in collaboration with the interdisciplinary care team. Coordinate community and social service referrals, including financial assistance, housing, transportation, and behavioral health resources. Collaborate closely with physicians, nurses, therapists, and case managers to support patient-centered, holistic care plans. Maintain accurate, timely documentation in electronic health records while complying with HIPAA and all regulatory standards. Adapt quickly to new practice environments, workflows, and team structures as you move between travel assignments. Advocate for patient rights, ethical care, and access to appropriate services in alignment with professional social work standards. Participate in ongoing professional development and utilize evidence-based practices to enhance patient outcomes.
