Job Description
Education, Experience and Certifications Education:
Master's degree in Social Work (MSW) requiredLicensure:
LCSW preferred; applicable state certification preferredProfessional Standards:
Adherence to the National Association of Social Workers (NASW)Code of Ethics Experience:
At least 1 year of professional experience in a hospital or healthcare-related setting preferredTechnical Skills:
Experience with data management tools.The ideal candidate will have:
Hospital or acute care management experience preferred Strong assessment, care coordination, and discharge planning skills Ability to work independently and collaboratively within an interdisciplinary team Strong communication skills and adaptability in a fast-paced environment Experience with complex discharge planning Flexibility and reliability for PRN coverage needsBenefits
