Job Description

DocCare is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our multidisciplinary team. The LCSW will provide therapeutic services, and support patients with complex needs. This is a part-time position working up to 20 hours per week. This position is for our location in Hauppauge, NY. The ideal candidate will have a strong commitment to patient advocacy, clinical excellence, and a holistic approach to health. This is an outstanding opportunity for the candidate to make a difference to the patients in a high-demand neighborhood.
Responsibilities:
1. Conduct mental health assessments to evaluate patients' mental health, social, and environmental needs. 2. Develop, implement, and monitor individualized treatment plans in collaboration with patients, families, and healthcare providers. 3. Provide evidence-based therapy, including individual, group, and family counseling. 4. Coordinate care with other healthcare professionals, including primary care providers, psychiatrists, and community resources. 5. Educate patients and their families on mental health conditions, coping strategies, and available resources. 6. Maintain accurate and timely clinical documentation in compliance with company policies and state regulations. 7. Participate in case consultations, team meetings, and training sessions to support collaborative care efforts. 8. Advocate for patients and connect them with community services, such as housing, financial aid, or support groups, as needed. 9. Other related duties as assigned.
Education:
Master's degree in social work (MSW) required. Minimum 2 years of experience in a healthcare or behavioral health setting.
Minimum Requirements:
Licensed clinical social worker (LCSW) in the state of practice required. Must have proficiency in evidence-based therapeutic modalities (e.g., CBT, DBT, trauma-informed care, etc.). Strong interpersonal and communication skills required. Must have the ability to work independently and as part of a multidisciplinary team. Experience working with diverse populations and addressing health disparities needed. Must have experience working with specific populations, such as children, adolescents, adults, families, or older adults.
Preferred Qualifications:
5 years of experience strongly preferred. Familiarity with electronic medical records (EMR) systems and telehealth platforms is a plus. Bi-Lingual strongly preferred.
