This part time role includes developing individualized care plans, addressing adjustment to chronic illness, and coordinating resources such as transportation, financial assistance, assisting with insurance issues and community support programs. The LCSW will collaborate closely with nephrologists, nurses, and other interdisciplinary team members to promote holistic, person-centered care. Daily responsibilities include case management, crisis intervention, facilitation of support and education, and documentation in accordance with regulatory and organizational standards.
