A Mental or Behavioral Health Case Manager coordinates overall plan of care for patients with mental or behavioral health needs. Oversees patient evaluation and treatment planning and monitors patient progress. Coordinates with the rest of the mental health team, including doctors and nurses, to establish these plans.

Position Summary We are seeking an experienced and dependable Case Manager to provide hands-on leadership and oversight of direct care staff and program services for adults with developmental disabilities. The Case Manager plays an important role in maintaining client safety, quality service delivery, accurate documentation, staff accountability, and regulatory compliance. This position works closely with direct care staff throughout the day, providing real-time coaching and guidance while ensuring client-specific plans, behavioral support, safety protocols, and documentation requirements are consistently followed. This is an ideal opportunity for someone who combines strong case management and documentation skills with hands-on staff leadership and client support experience.
What You'll Do Lead & Develop Staff:
Provide daily direction, observation, coaching, training, and performance feedback to direct care employees. Support new employee orientation and ongoing competency development while reinforcing professional, person-centered services.
Provide Behavior & Client-Support Guidance:
Coach employees on implementing client-specific behavioral supports, supervision requirements, risk protocols, and established intervention strategies. Provide guidance during challenging situations and help staff respond calmly, safely, and consistently to client needs.
Coordinate Staffing & Performance:
Assist with staffing coverage, call-outs, schedule changes, and staff-client assignments. Monitor employee attendance, conduct, performance, and compliance with established expectations and address concerns through coaching, documentation, or corrective action as appropriate.
Manage Client Documentation & Records:
Review progress notes, ISP and behavior data, communication logs, body checks, incident reports, and other client records for accuracy, completeness, timeliness, and objectivity. Maintain client charts, electronic records, health plans, tracking tools, and required program documentation.
Support Client Safety & Risk Management:
Ensure employees understand and follow client-specific medical, dietary, behavioral, supervision, restroom, choking-risk, and other identified safeguards. Maintain awareness of changing client needs and promptly communicate concerns. Respond to
Incidents & Emergencies:
Provide direction during behavioral, medical, safety, and operational incidents. Ensure emergency procedures are followed and complete required incident documentation, notifications, reporting, and follow-up within established timelines.
Support Case Management & Program Planning:
Review client data and service trends, assist with semiannual and annual progress reporting, participate in interdisciplinary person-centered meetings, and support client referral and admission activities when assigned.
Oversee Community & Transportation Activities:
Monitor community outings, transportation requirements, driver and vehicle documentation, and staff-client assignments to ensure appropriate supervision and compliance with established safety procedures.
Communicate & Collaborate:
Promptly escalate significant client, staffing, documentation, or safety concerns and maintain professional communication with employees, clients, BCBA, nursing staff, management, and other stakeholders. Sound judgment when responding to behavioral, medical, safety, or operational emergencies. Ability to maintain confidentiality, professional boundaries, and objective documentation. Ability to collaborate effectively with staff, clients, families, management, behavioral professionals, nursing staff, and service coordinators. Qualifications Bachelor's degree in psychology or another Human Services field with 1 year of management experience working in a day program serving adults with developmental disabilities and behavior management. Or 3 years of management experience working in a day program serving adults with developmental disabilities and behavior management. Valid California Driver License and acceptable driving record. Vehicle insurance and access to an approved vehicle when driving is an assigned job function. Ability to complete required fingerprint/background clearance, drug test, physical, and TB test. Work Environment This is an active position that works across programs, transportation, and community settings. The Case Manager must regularly move throughout the program to observe clients and employees and be prepared to respond to behavioral, medical, and operational situations.
