Our Integrated Care department seeks to reduce the premature mortality of individuals living with mental illness by providing chronic disease prevention and wrap-around chronic disease management services. This specialty care team helps clients build greater independence, reconnect with their communities, and access treatment in the least restrictive setting appropriate for them, while recognizing that clients living with complex or chronic medical conditions sometimes need us to bring care to them. The Integrated Care Senior Case Manager provides administrative and clinical supervision to direct staff, as well as care planning, resource identification, benefit coordination, service monitoring, and community-based counseling support to assist clients with improving symptoms and behaviors that otherwise interfere with daily functioning and wellness goals. This is a full time, 40 hour per week position and is considered exempt in accordance with the Fair Labor Standards Act (FLSA). Major Functional Areas and Description of Duties Train all new and seasoned case managers in the Integrated Care department on: case management notes, case management assessments, Care Plan writing, Medicaid benefits and related changes, resources and resource updates throughout the year, treatment planning, Adult Needs and Strengths Assessments (ANSA), crisis planning, Functional Support Services, documentation, daily attestation of notes, and weekly attestation of care plans. Provide formal, regularly scheduled case management supervision to assigned staff including review of cases and weekly/monthly productivity reports; Provide back-up supervision and support to other case managers in absence of Case Management Supervisor; Manage time off requests and weekly payroll for designated staff; Working in conjunction with Integrated Care Case Management Supervisor to ensure that service delivery is efficient and effective by developing methods to meet desired client outcomes, productivity, and budget variances; Review productivity (monthly case management and weekly functional support services) and problem solve with staff to meet expectations; Provide ongoing feedback on performance and complete formal performance reviews; Provide 10 case management contacts per month (eligible for incentive pay of $40 per case management contact upon exceeding productivity expectation); Provide 4 hours of billable service each week to meet 10% of direct service key performance indicator (eligible for incentive pay upon exceeding productivity expectation at a rate of $40 per hour); Participate in leadership and team meetings; Promote a positive work environment and culture that fosters cooperation, teamwork, professional growth and development to benefit CLM staff, clients, families, and community partners; Ensure self and team members: Provide individual assessment to create treatment and care plans that specify presenting problems, objectives, interventions, and desired outcomes; Assist clients with applying for and maintaining adequate medical coverage to support consistent access to whole health care and treatment; Provide outreach, advocacy, coordination, resource identification, linkage and monitoring of internal and external supports and services; Provide counseling and Functional Support Services (FSS) in the home or other community-based setting as deemed appropriate for the objectives, interventions and desired outcomes identified in the client's treatment plan; Provide Individual Therapeutic Behavioral Services (TBSI) to develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement; Provide Crisis Intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community; Work collaboratively with other treatment team members, Healthy Together team, natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care; Serve on appropriate committees, both internal and external, as determined by supervisor; Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients; Execute all required services and timely documentation in accordance with agency policies and federal/state standards; Maintain professional memberships, licenses, accreditation, certifications, etc.; Represent CLM in a professional and appropriate manner in all settings; Perform all other duties as assigned by supervisor. Physical Demands and Working Conditions The work will be primarily community-based to include visiting clients in their homes and in their environment. Additionally, work will be performed in the office and remote via telehealth. Manual dexterity is needed to use computers and standard office equipment. The ability to sit for extended periods of time is required. Reliable internet for remote work is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required for this role. This is an exciting opportunity for CLM staff to earn a Master of Science in Clinical Mental Health Counseling (MS) degree in 3 years.
Who's Eligible and What's Required:
Must be a regular, full-time CLM employee in good standing for 9 months or more. Must hold a Bachelor's Degree. Must agree to a five (5) year commitment of full-time employment in good standing with the Center for Life Management (beginning from start date/year of Professional Development Program participation). Participants must maintain a B average to continue participation in the program. Requirements:
Qualifications and Skills Bachelor's degree preferred however an Associate's degree may qualify; Two years of full-time clinical experience required; Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations; Proficient computer skills and knowledge of all Microsoft office applications (Word, Excel, Publisher, Power Point, Outlook, TEAMS, etc.); Ability to document effectively and efficiently in course of clinical work; Ability to navigate an electronic health record; Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision; Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services; Effective communication skills, organizational skills, and ability to multi-task.
