Millennium Community Services (MCS) is a psychiatric outpatient provider committed to delivering quality, community-based mental health and rehabilitative services to individuals and families through counseling, education, advocacy, and coordinated care. MCS is currently seeking a compassionate and dependable Case Manager I or II to join our team at our Poteau Clinic . The Case Manager will work closely with clients, families, providers, and community resources to help clients access needed services, achieve treatment goals, and improve overall functioning and well-being. Duties and Responsibilities Assess clients' needs and develop individualized service plans. Coordinate care and connect clients with appropriate community resources and services. Monitor clients' progress and provide ongoing support toward their treatment and wellness goals. Advocate for clients and assist them in accessing needed behavioral health, medical, and social services. Collaborate with clinical providers, families, and other members of the treatment team. Assist with referrals, appointments, scheduling, and coordination of care. Identify and report client safety or crisis concerns to appropriate clinical staff. Maintain accurate and timely documentation in the Electronic Health Record (EHR). Complete required case notes, forms, and reports within established timelines. Maintain client confidentiality and follow HIPAA, agency policies, and applicable regulations. Work collaboratively with the team to meet client and organizational goals. Perform other duties as assigned within the scope of the position. Preferred Qualifications Previous experience in behavioral health, social services, community mental health, or case management. Experience with EHR documentation and community-based service coordination.

Pay:

$16.00 - $27.00 per hour
