A Mental or Behavioral Health Case Manager coordinates overall plan of care for patients with mental or behavioral health needs. Oversees patient evaluation and treatment planning and monitors patient progress. Coordinates with the rest of the mental health team, including doctors and nurses, to establish these plans.

WHAT YOU WILL DO
───────────────────────────── Deliver individual and group skills training sessions to children ages 5-15 Provide targeted case management services including coordination with schools, families, and community resources Facilitate group psychosocial rehabilitation sessions in clinic, school, and community settings Conduct in-home visits and community-based sessions as needed — mileage considered Document sessions accurately and timely in our EHR system Collaborate with supervising LPC on treatment plan goals and clinical progress Participate in occasional telehealth sessions for case coordination and parent consultations ─────────────────────────────

───────────────────────────── Bachelor's degree in Psychology, Social Work, or a related behavioral health field Bilingual — English and Spanish strongly preferred given our local community Minimum 1 year of experience in mental health, case management, or direct service with children or adolescents Meets Texas Medicaid QMHP criteria — able to provide documentation upon request Experience with or willingness to learn case management practices and documentation Valid Texas driver's license and reliable transportation — in-home visits required Ability to pass a background check and OIG exclusion screening
