Position Overview:

Works as a part of a multidisciplinary primary care treatment team to facilitate the provision of holistic, whole-person healthcare for all Kintegra Health patients. Provides behavioral health and substance abuse screening, brief intervention, education, and referral services for patients presenting in Kintegra Health primary care clinics. Provides ongoing consultation and treatment planning support for primary care medical staff in their work with patients with psychosocial, behavioral health, chronic disease, behavior change, and/or substance use concerns. Is knowledgeable of and adheres to all policies, professional practice standards, philosophies and requirements for professional clinical practice.

Job Duties:

Practice reflects adherence to the professional Code of Ethics. Provides evidence-based assessment, intervention, consultation, and referral services for patients with a variety of behavioral and chronic health conditions Works cooperatively as an integral part of the primary healthcare team with the goal of improving patient care and increasing team efficiency. Formulates and facilitates the implementation of behavioral health, substance abuse, and chronic disease management plans of care through use of appropriate treatment modalities, interventions, and available community resources. Assists primary care providers with screening, identification, diagnosis, management, and treatment of behavioral health, substance use, issues, chronic disease management Engages in ongoing assessment and quality improvement strategies to assist in the implementation of ongoing primary care/behavioral health integration strategies. Provides consultative services to health care team members within the scope of care definitions and provides training to other health care professionals as needed. Completes, in a timely manner, all required documentation related to patient treatment, payment for services rendered, and organizational operations. Engages in continuing education to maintain licensure and enhance professional competence relevant to medical and legal issues that impact patient care. Maintains working knowledge of and liaison with community agencies and resources. And other duties as assigned

Requirements:

Education:

Master's Degree Experience:

Minimum two years' experience in a public health setting.

Professional Licensure:

Current, unrestricted South Carolina licensure as a Licensed Independent Social Worker-Clinical Practice (LISW-CP), in good standing. Certification(s):

None Minimum Qualifications:

Master's degree in social work from a program accredited by the Council on Social Work Education (CSWE). Current, unrestricted South Carolina licensure as a Licensed Independent Social Worker-Clinical Practice (LISW-CP), in good standing. Experience providing behavioral health services in an outpatient, primary care, or integrated-care setting. Knowledge of age-appropriate care and quality-improvement practices. Ability to maintain patient confidentiality, comply with HIPAA and professional ethical standards, and communicate effectively with patients, families, providers, and employees. Current Basic Life Support certification.

Additional Skills Required:

Proficiency in using electronic health record systems to document patient care and update patient information. Knowledge of basic medical and behavioral health terminology. Strong verbal and written communication, organization, problem-solving, and critical-thinking skills. Ability to manage changing priorities, exercise sound professional judgment, maintain confidentiality, and work effectively as part of a multidisciplinary team. Ability to communicate respectfully and effectively with patients, families, employees, and other individuals from diverse backgrounds.

Physical Requirements:

This position is primarily sedentary and requires extended periods of sitting during patient appointments and while completing documentation. The role also requires occasional standing and walking within the clinic and frequent use of a computer, telephone, and other standard office equipment. Occasional travel to other clinic or community locations may be required. A qualified individual must be able to perform the essential functions of the position, with or without reasonable accommodation.

Proficiency in the electronic health record system and other computer applications used within the practice.
