Position Summary Proactive Hospice Care is seeking a compassionate and experienced Hospice Chaplain & Bereavement Coordinator for a part-time, contingent, per-diem position. This role provides spiritual care, emotional support, and bereavement services to hospice patients and their families in alignment with their individual beliefs and preferences. This position is paid per visit, with compensation for initial spiritual assessments and follow-up (re-visit) encounters.
