We are seeking a Resident Services Assistant to support our Resident Services Coordinator and help residents and families navigate admissions, appointments, and discharge planning. As a Resident Services Assistant, you will play a vital role in keeping important details organized while providing residents and families with a welcoming, helpful point of contact.

Responsibilities:

Assist the Resident Services Coordinator with processing admission paperwork and meeting with residents and families to gather required information. Help complete and document social services assessments, including follow-up throughout a resident's stay. Support discharge planning by helping coordinate paperwork, services, resources, and communication with residents and families. Schedule and manage resident appointments, including communicating appointment details and coordinating transportation as needed. Work with nurses, therapists, and other team members to address resident needs and support smooth transitions of care. Participate in care conferences and help follow up on resident and family questions or concerns. Maintain accurate, timely documentation in accordance with facility policies and applicable requirements. Advocate for residents' rights, preferences, and dignity in every interaction. Keep the Resident Services Coordinator informed of upcoming appointments, discharge needs, incomplete paperwork, and concerns requiring follow-up.
