This includes coordinating with administration, medical, and nursing staff to ensure effective service delivery. Key responsibilities involve developing and reviewing departmental policies and procedures annually, with a focus on identifying residents' social and emotional needs. The professional also participates in community planning to align services with resident and family needs. Additionally, they engage in discharge planning and create social care plans through private interviews with residents and families. Administrative tasks, such as completing forms and reports, are essential, as is involving residents and families in program planning. Lastly, the professional arranges transportation to other facilities as needed, ensuring seamless care.
