Job Summary Elevated Abilities Support Services is seeking a dependable, organized, and compassionate Waiver Support Coordinator to join our team. The Waiver Support Coordinator is responsible for coordinating and monitoring services and supports for individuals receiving waiver services. This position works closely with individuals, families, guardians, service providers, and other members of the support team to help ensure services are properly coordinated, documented, and delivered according to each individual's needs and approved service plan. The ideal candidate is highly organized, professional, detail-oriented, and committed to providing person-centered support to individuals with intellectual and developmental disabilities. Key Responsibilities Coordinate and monitor waiver services and supports based on each individual's approved service plan. Maintain regular communication with individuals served, families, guardians, providers, and other members of the support team. Assist with identifying individual needs, goals, resources, and appropriate community-based supports. Monitor authorized services and follow up on service-related concerns or changes. Maintain accurate, complete, and timely case notes, reports, service records, and required documentation. Schedule and participate in meetings, assessments, reviews, and person-centered planning activities. Assist individuals and families with understanding available services, resources, and waiver requirements. Coordinate with service providers and other professionals to promote continuity and quality of services. Identify changes in an individual's needs, services, or circumstances and communicate concerns to the appropriate parties. Assist with resolving service gaps and coordinating referrals to appropriate resources. Maintain confidentiality and protect individuals' personal and health information. Follow applicable Medicaid waiver requirements, agency policies, and state and federal regulations. Meet required documentation and reporting deadlines. Complete required training and maintain applicable credentials. Perform additional duties related to support coordination as assigned. Required Qualifications College degree required in social work, human services, psychology, sociology, healthcare, education, behavioral sciences, or a related field. Experience working with individuals with intellectual and developmental disabilities, waiver programs, case management, support coordination, healthcare, or human services preferred. Strong written and verbal communication skills. Excellent organizational, time-management, and documentation skills. Ability to manage multiple individuals, deadlines, appointments, and service requirements. Strong computer skills and ability to learn electronic documentation and case-management systems. Ability to work independently and collaborate professionally with individuals, families, providers, and other professionals. Must demonstrate professionalism, reliability, sound judgment, and respect for individual rights, choices, and confidentiality. Valid driver's license and reliable transportation may be required. Must successfully complete all required background screenings and employment requirements. Preferred Qualifications Experience with Medicaid Home and Community-Based Services (HCBS) waiver programs. Experience with person-centered planning and service coordination. Knowledge of community resources and services available to individuals with disabilities. Experience preparing case notes, service plans, assessments, reports, and compliance documentation.
