Hamden, CT Job Summary:

We are seeking a highly skilled and compassionate Director of Social Services to join our team at Arden House in Hamden, CT. As a Director of Social Services, you will work closely with patients/residents and their families to provide exceptional social work services, ensuring their physical, mental, and psychosocial well-being. You will lead a comprehensive Social Services program, promote patient/resident rights, and collaborate with our interdisciplinary team to deliver high-quality care.

Responsibilities/Accountabilities:

Administrative:

Assist with planning and implementing a comprehensive Social Services program, ensuring compliance with federal and state regulations.

Advocacy:

Promote and protect patients/residents' rights, prevent and report abuse, and support patients/residents and families in self-advocacy.

Clinical:

Conduct comprehensive Psychosocial Assessments, develop and implement person-centered care plans, and provide therapeutic interventions to address patients'/residents' social, emotional, and psychological needs.

Discharge Planning:

Identify discharge goals, develop and implement discharge plans, and educate patients'/residents' families on health care decision making and advance directives.

Education:

Educate staff, patients'/residents, and families on the role of Social Services, patients'/residents' rights, and community resources.

Requirements:

Master's or Bachelor's degree in Social Work or Human Services field required. 1-3 years of supervised social work experience in a healthcare setting preferred. Additional certifications such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/ or Substance Use preferred.
