Acts as a liaison between any necessary state, federal, or local agencies, finances; or with families and guardians on behalf of Woodhouse, Inc. Ensure residents Bill of rights are being adhered to.
POSITION DUTIES
Social Services 1. Document all contact with families/guardians, including phone calls, letters, invitation to events, etc. 2. Orchestrate quarterly family tea gatherings and Ethnic Nights in coordination with the Dietary and Residential Departments. 3. Coordinate therapeutic visits with family and friends with the Medical and Residential Departments. 4. Coordinate emergency situations involving consumers (i.e.: burial/funeral needs or a family crisis of a resident). 5. Responsible for coordinating admissions and discharges of consumers. 6. Serve as liaison between Woodhouse, Inc. and government agencies or the families and guardians of our consumers. 7. Organize, delegate and monitor quarterly Consumer Rights Reviews, writing a quarterly summary. 8. Facilitates and oversee annual Consumer Government elections. 9. Process Representative Payee Reports, SSI, Food Stamps, SSA, and VA paperwork. 10. Chair the Human Rights committee and maintain minutes. 11. Oversee all abuse allegations practicing highest standards of confidentiality. 12. Oversee orientation for all new employees. 13. Ensuring that all case files, and other records, strictly comply with policies, regulations, and procedures 14. Responsible for recruiting and coordinating placement of new clients. Staff Training 1. Oversee all Social Work training.
POSITION REQUIREMENTS EXPERIENCE
Experience in training D/D clients and staff a plus. Excellent organizational skills, excellent writing skills, and effective verbal communication. Ability to work independently and function as part of the Interdisciplinary treatment team.
EDUCATION
Minimum of a bachelor's degree in psychology, Social Work or human service.

PHYSICAL DEMANDS
This position requires a moderate amount of physical work. Occasional lifting generally not to exceed 50 pounds required to lift, transfer and reposition residents to and from wheelchairs, chairs, bus seat, etc. Assist with emergency physical intervention of residents who are posing an immediate danger to themselves, others, or to property. Travel into the community for professional meetings and events. Travel to visit other organizations and agencies.
Pay:
$50,000.00 - $55,000.00 per year
