Job Description

We are seeking a compassionate, organized, and dedicated Social Services Director to join our team at our skilled nursing facility in Florida. This is an excellent opportunity for an experienced long-term care professional who is passionate about resident advocacy and is looking to grow into a leadership role. Position Summary The Social Services Director is responsible for assisting residents and their families with psychosocial needs, discharge planning, care planning, and accessing community resources. The ideal candidate has experience working in a nursing home and is committed to enhancing the quality of life for our residents. Qualifications Experience working in a long-term care or skilled nursing facility is required. Previous experience in social services, admissions, case management, or a related healthcare role is preferred. Knowledge of resident rights, discharge planning, and Medicare/Medicaid processes is a plus. Excellent communication, interpersonal, and organizational skills. Ability to work collaboratively with an interdisciplinary team. Must meet Florida requirements for a non-qualified Social Services Director in a licensed nursing facility. Responsibilities Assess residents' psychosocial needs upon admission and throughout their stay. Participate in interdisciplinary care plan meetings. Assist residents and families with adjustment to placement and ongoing care. Coordinate discharge planning and referrals to community resources. Advocate for resident rights and quality of life. Maintain accurate and timely documentation in accordance with state and federal regulations. Work closely with nursing, therapy, admissions, and administration to ensure comprehensive resident care. Assist with grievance resolution and family communication.
