Key Responsibilities:

Act as an advocate for residents, ensuring their best interests are prioritized and protected. Assess the psychosocial status of residents in relation to their illness and environment. Communicate assessment findings effectively to the registered nurse and interdisciplinary team. Conduct social evaluations and develop person-centered intervention plans based on evaluation results. Maintain accurate and comprehensive clinical documentation in accordance with facility policies and regulatory requirements. Collaborate with residents, families, and healthcare team members to coordinate appropriate social services and support. Assist residents in accessing community resources, benefits, and other support systems. Ensure compliance with all background check requirements and facility policies prior to contact with residents. Requirements:

Education Bachelor's or Master's degree in social work. Licenses/Certification LCSW, LSW, or MSW license required. Must have a valid driver's license. Experience Experience in post-acute/ skilled nursing preferred.
