Connect clients to community resources for housing, financial literacy, and legal/estate planning needs; refer to licensed professionals when appropriate. Work with government agencies and community organizations to stay current on available benefits, policies, and regulations. Manage a team supporting MAPD clients across all health and insurance needs, including chronic disease monitoring, hospitalizations, and prescriptions. Track client cases and follow through to ensure under-privileged individuals and families receive the support they need. Train and mentor team members on social service delivery and compliance with program guidelines. Prepare program evaluations documenting outcomes and areas for improvement.
