$85,000 Annually Make a Difference Every Day Acacia Care Rehabilitation and Nursing Center is seeking a compassionate, experienced, and dedicated Social Services Director to join our leadership team. In this key role, you will oversee the facility's social services department, ensuring residents and their families receive the emotional, psychosocial, and practical support needed to enhance quality of life and achieve the best possible outcomes. Key Responsibilities Direct and oversee all social services programs and services within the facility Coordinate discharge planning, financial assistance, and community resource referrals Ensure psychosocial and behavioral health needs are addressed through individualized care planning Advocate for resident rights and provide education regarding advance directives and available services Participate in resident and family meetings to support communication and care planning Maintain knowledge of federal, state, and local regulations governing long-term care facilities Develop and update policies and procedures related to social services operations Collaborate with interdisciplinary teams to improve resident outcomes and quality of life Support regulatory surveys, audits, and compliance initiatives Establish and maintain relationships with community agencies and healthcare resources Provide support and guidance to residents, families, and staff regarding social service concerns Maintain accurate documentation in accordance with facility and regulatory requirements Qualifications Bachelor's degree required; Social Work or related field preferred Minimum of two (2) years of social work experience in a healthcare or long-term care setting Prior supervisory or leadership experience preferred Strong knowledge of skilled nursing facility regulations and resident rights Excellent communication, leadership, and organizational skills Ability to work effectively with residents, families, staff, and community partners Compassionate, professional, and resident-focused approach to care Must meet all applicable health and employment requirements Benefits & Perks Generous Paid Time Off (PTO) Medical and Dental Insurance Employee Recognition Programs Company Perks and Discounts Supportive, Team-Oriented Work Environment Opportunities for Professional Growth and Development Join Our Team If you're ready to lead with compassion and make a lasting difference in the lives of residents and families, we encourage you to apply today.
