Role Overview As the Community Liaison East Valley, you will build and maintain referral relationships that connect patients and families to home health services across the East Valley. This role exists to expand community awareness, support census growth, and ensure a smooth, timely referral-to-admission process. Key responsibilities include developing relationships with hospitals, physician offices, case managers, and community partners; educating referral sources on services and eligibility; tracking and following up on leads; coordinating with intake/clinical teams to remove barriers; documenting outreach activity and results; and representing the organization at meetings and events.
Required qualifications/skills:
Experience in healthcare sales, home health, hospice, or related referral development Strong communication, organization, and follow-through Working knowledge of referral workflows and compliance expectations Valid driver's license and ability to travel locally Apply today to be considered for this full-time, onsite opportunity in Tempe. A Typical Day Your day is spent out in the East Valley connecting with referral partners, balancing scheduled touchpoints with responsive follow-ups. You'll plan your route, confirm meetings, and adjust priorities as new opportunities arise. Between visits, you update activity notes, review progress toward outreach goals, and prepare concise materials for upcoming conversations. You'll also stay closely aligned with internal teams to understand current capacity and service updates, so you can set clear expectations with partners. By late afternoon, you wrap up documentation, confirm next-day appointments, and organize your pipeline to keep momentum moving forward.
