The CS Housing Specialist is responsible for delivering housing-related support services to Medi-Cal beneficiaries who are homeless, at risk of homelessness, or transitioning from institutions under the CalAIM initiative. The CS Housing Specialist helps client's access and maintain stable housing by providing housing navigation, tenancy support, and coordination with community partners. This position works collaboratively with Enhanced Care Management (ECM) teams, community-based organizations, landlords, and housing agencies.
Responsibilities:
1. Conduct housing needs assessments and assist clients in developing individualized housing plans. 2. Provide Housing Transition Navigation Services , including assistance with housing applications, documentation collection, and housing searches. 3. Deliver Housing Tenancy and Sustaining Services such as landlord mediation, education on tenant responsibilities, and housing stability support. 4. Assist clients in obtaining necessary documentation (e.g., ID, income verification) and accessing public benefits related to housing. 5. Build relationships with local landlords and property managers to identify housing opportunities. 6. Maintain knowledge of local housing programs, subsidies, and eligibility requirements. 7. Monitor and document client progress, service delivery, and outcomes using electronic health records (EHR) and/or case management systems. 8. Participate in multidisciplinary care team meetings to coordinate services and ensure continuity of care. 9. Support clients during move-in processes, including coordination of transportation and furniture delivery when necessary. 10. Provide crisis intervention and support for housing-related emergencies or disputes. 11. Comply with all program regulations, documentation requirements, and quality assurance standards.
Qualifications:
1. High school diploma or equivalent (GED) required; associate degree or coursework in human services, social work, or public health preferred. 2. Minimum 2 years of experience in case management, housing navigation, or social services, particularly with populations experiencing homelessness, serious mental illness, substance use disorders, or other complex needs. 3. Familiarity with CalAIM Housing Services, Medi-Cal eligibility, and housing systems (e.g., Coordinated Entry, Section 8, rapid rehousing, permanent supportive housing). 4. Understanding of local community resources and service systems. 5. Strong interpersonal, written, and verbal communication skills. 6. Proficient in data tracking, reporting, and use of electronic health records (EHR). 7. Competency in Microsoft Office and electronic client record systems. 8. Strong organizational, analytical, and project management skills. 9. Ability to work independently and collaboratively in a team-based, fast-paced environment. 10. Able to manage multiple priorities and provide staff support when needed. 11. Culturally responsive, especially within Native American communities; strong customer service and ability to work with individuals from diverse backgrounds. 12. Valid driver's license, clean driving record, and willingness to travel. 13. Able to sit, stand, walk, and drive for extended periods; lift up to 25 pounds. 14. Willingness to conduct frequent home visits, including in remote or rural areas. 15. Must pass a pre-employment drug screening. 16. Native American preference will be given in accordance with
