Position:
Qualified Professional We are seeking a compassionate, knowledgeable, and dedicated Qualified Professional to join our team. This role is ideal for someone who is passionate about supporting individuals with mental health and developmental disabilities and who values person-centered, community-based care. The Qualified Professional will help coordinate services, support individualized treatment planning, monitor client progress, and collaborate with individuals, families, and service providers to promote positive outcomes. Key Responsibilities Conduct comprehensive functional assessments and reassessments to support clinical decision-making and individualized treatment planning. Develop individualized plans of care and person-centered plans that include measurable goals, objectives, and desired outcomes. Implement, monitor, and update service plans while coordinating required authorizations for home- and community-based services. Respond to and resolve client concerns to promote satisfaction and ensure quality service delivery. Collaborate with internal staff and external providers to coordinate services and strengthen continuity of care. Coordinate client discharges, transfers, and service terminations in accordance with established procedures. Ensure services are delivered in compliance with agency policies, ethical standards, professional guidelines, and applicable regulations. Participate in case consultations and collaborate with therapists and other members of the individual's care team. Maintain accurate, complete, and timely client records and service documentation. Oversee Individualized Supervision Plans in community-based and facility-based settings, as applicable. Coordinate services with community mental health and intellectual/developmental disability organizations. Attend required trainings, staff meetings, and in-service sessions. Perform other duties as assigned and outlined in applicable Service-Specific Definitions. Required Skills and Competencies The ideal candidate will demonstrate: Strong knowledge of local resources and service systems available to children, adolescents, adults, and older adults with mental health needs. Experience conducting assessments, developing service plans, and monitoring service delivery. Knowledge of crisis intervention techniques and the ability to respond effectively in high-stress situations. Excellent written and verbal communication skills. Ability to work independently in community settings while collaborating effectively with a multidisciplinary team. Ability to work respectfully and effectively in diverse and multilingual environments, including working with interpreters when needed. Proficiency in maintaining accurate paper-based and electronic records. Strong computer skills, including proficiency with Microsoft Office and related software. Effective organizational and time-management skills, with the ability to prioritize tasks and meet deadlines. Willingness and ability to travel to client homes, including locations that may not be ADA accessible. Qualifications Minimum education and experience requirements include one of the following: Bachelor's degree in a non-human services field with four (4) years of relevant
MH/DD/SA
experience. Bachelor's degree in a human services field with two (2) years of relevant experience. Master's degree in a human services field with one (1) year of relevant experience.
