License Carmel, NY Apply Job Type Full-time Description Case Manager (CASAC-T, CASAC, LMSW, LMHC, LCSW ) Arms Acres is looking for a Clinical Case Manager to join our team in Carmel, NY. In this role, you will provide comprehensive clinical case management services that include: assessments, individual, group and family treatment, treatment plan coordination, referent relations, discharge planning, and intervention with client support systems, including social service programs and managed care. •sign on bonus eligible
DUTIES AND RESPONSIBILITIES
Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures. Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames. Documents patient progress and own interventions in case record and works within time frames of regulatory requirements. Provides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program director. Performs utilization review, provides progress reports and develop aftercare plans in conjunction with referring persons/agencies, EAPs and managed care/insurance entities. Participates in facility program and quality improvement activities. Requirements
REQUIRED QUALIFICATIONS
